Job Description:
U.S. Renal Care is seeking a dedicated Nurse Practitioner to
join our Value-Based Care team in San Jose. This role is pivotal in
providing exceptional and personalized care to patients with kidney
disease, working closely with multidisciplinary teams to manage and
improve patient outcomes. The successful candidate will enjoy a

up to $15,000 is available for highly qualified candidates. Assess
patient health through interviews and physical examinations.
Collaborate with other healthcare professionals to plan, manage,
and assess patient treatments. Prescribe medications and treatments
judiciously. Monitor patient outcomes and adjust treatment plans as
necessary. Provide education and support to patients and their
families regarding kidney health and related lifestyle adjustments.
Participate in research and outreach initiatives designed to
improve patient outcomes. Ensure compliance with healthcare
regulations and clinical standards. Expert knowledge of nephrology
and chronic disease management. Strong clinical skills in patient
assessment and diagnosis. Proficient in using healthcare software
and systems. Ability to communicate effectively with patients,
families, and medical teams. High level of empathy and
supportiveness. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Valid Nurse Practitioner license. Board
Certification in Nurse Practitioner specialty (e.g., Family Nurse
Practitioner, Adult-Gerontology Nurse Practitioner). CPR
certification.
